This policy describes how FDTEC — Endocrinology & Metabolic Wellness handles information collected through this website, fdtec.org.

It is notthe practice's Notice of Privacy Practices. Protected health information created or received in the course of your care is governed by HIPAA and by the Notice the practice provides at your visit. Ask the front desk for a copy, or call (407) 574-5161.

Information we collect

Information you give us. The appointment request form on this site asks for your name, phone number, and an optional reason for your visit. Please do not include detailed medical information in that form — it is a scheduling request, not a secure clinical channel.

Information collected automatically. When you submit the form we also record the page you submitted from, the site that referred you, and any campaign parameters in the link you followed. This tells us which of our pages patients find useful.

Analytics. We use Google Analytics to understand how the site is used in aggregate. Analytics data is not linked to your medical record.

How we use your information

  • To call you back and schedule your appointment.
  • To answer the question you contacted us about.
  • To understand which pages help patients, in aggregate.

We do not sell your information, and we do not share it with advertisers.

When we share information

We share information only with the service providers that operate this site and deliver appointment requests to the office, and only to the extent they need it to do that work. We may also disclose information where the law requires it.

How long we keep it

Appointment requests are retained for as long as needed to schedule and document your care, and then in line with the practice's records-retention schedule and Florida law.

How we protect it

This site is served over an encrypted connection, and appointment requests are transmitted encrypted. No method of transmission over the internet is completely secure, so please use the phone for anything sensitive or urgent.

Your choices and rights

You can ask us what information we hold about you, ask us to correct it, or ask us to delete information that is not part of your medical record. Rights over your medical record itself are described in the practice's Notice of Privacy Practices.

You can opt out of Google Analytics using Google's browser opt-out add-on, and most browsers let you block cookies.

Children

This website is not directed to children, and we do not knowingly collect information from children through it.

Changes to this policy

If we change this policy we will update the effective date above.
